Please click on the Patient Registration form below. You will be able to fill it out online. Once completed you will be able to send it electronically directly into your record at our office. If you are not able to do so, please plan to arrive early for your appointment to fill out the paper forms.
Please be sure to bring any insurance eligibility cards and a complete list of medications that you are taking on a regular basis.
What To Expect
Our staff will review your completed registration form and insurance information. Dr. O’Niel will discuss your medical history and your list of medications. Be sure to let us know in advance if you need to take antibiotics before your appointment. We will take any necessary dental x-rays. You will receive a complete intraoral examination and oral cancer screening. Finally we will perform a dental prophylaxis (cleaning). Dr. O’Niel will then share the findings with you and discuss your treatment options.
